Before you begin this application be prepared to provide:

• A recent paystub reflecting at least 30 hours for two weeks (15 hours per week) 
• Your supervisor's name and work email 
• Prime Time Registry Update: after you submit your application, it is required that you login to the Prime Time Portal to review/update your personal, employment and training/education information (click the green confirmation button after you review). Your application will not be processed until this step is complete. Your mailing address must be accurate for your check to be processed correctly.
