Note

Before you begin this application be prepared to provide:

  • A recent paystub reflecting that you work at least 30 hours per two week pay period (15 hours per week).

  • Your supervisor's name and work email  

Registry Member ID Number
Please provide your Registry Member ID Number. If you forgot your Registry Member ID, contact Prime Time's Registry Coach, Elisa Moro.  Elisa can be reached at emoro@primetimepbc.org, 561-600-9528 or by clicking the live chat button.  

 

If you are not a registry member, click here to schedule a registry orientation.  Within two business days of completing the registry application, you will receive a welcome letter with your registry member ID (at that time you can apply for ACHIEVE OST).
